Need to refer a patient?

Most infertility concerns are resolved with conservative interventions such as clomid, insemination, or surgery.

Referral to a specialist is recommended when:

  • Female age of about 38 or older.
  • Blocked fallopian tubes at any age (1 or both tubes blocked).
  • Other tubal problems at any age - such as more than one tubal ectopic pregnancy.
  • Moderate or severe endometriosis at any age.
  • Significant male factor - sperm concentration less than 10 million per ml, or motility less than 40%.
  • Ovulation problems such as polycystic ovary syndrome, PCOS treated with 3 to 6 months of Clomid without conception at any age.
  • Abnormal ovarian reserve tests, such as elevated day 3 FSH level at any age.

In addition to advanced infertility consultations SRM accepts referrals for (other RE conditions), tubal reanastomosis, and vasectomy reversal.

How to Refer

Your patients have already waited a long time to become pregnant. SRM wants to make their intake for fertility treatment efficient so they can get started as quickly as possible.

  • Patients may call Centralized Scheduling at SRM directly to schedule a physician appointment. This number is (206) 301-5000.
  • Physician offices should send medical records to SRM via fax or provide to patient to bring at time of appointment. Bringing all prior medical records hastens the treatment planning phase.
  • After her initial consultation and at confirmation of viable pregnancy the referring physician will receive a written report with treatment plan and outcomes.
